perc6 managed raid, disks randomly going into "unknown" state

It's happened for us with some disks timing out.

Disk firmware updates have helped similar issues in the past
(almost every disk vendor has experienced similar problems, with
 many different RAID cards - it's an endemic problem not specific
 to any RAID card or disk drive manufacturer).
