megaraid problem on PERC 4/DC, Red hat ES 3 and PV220S: mount los t!

5126A seems quite old. You should upgrade to 521X as there are fixes
for problem relating to RAID 5.
Being PERC4/DC as you said I think 351X is more appropriate.

Anyway, prepare to have a lot of troubles (or at the very least poor
performance) when you have a voulme > 4 X 146 GB hard drives in RAID 5
configuration ( > 450 GB).
