independent disk on a PERC 3DCL (PE1550) keeps going offline.

We have a server whose support I've inherited that every few days has 
one of its drives go offline. A reboot and using one of the utilities 
on startup to manually bring it on-line makes it happy for a week or 
so.

The disk configuration is 2 drives as RAID 1 mirrored set, and a 
later expansion drive added to the PERC 3 as an independent disk. 
This is of course the drive that goes off-line.

I've not found anything leaping out of /var/log/messages or the boot 
log, and I don't think my predecessors installed any of the Dell 
utilities in their installation (SuSE 9.3).

Where should I be looking for more detailed reporting (if it exists)? 
Can I find it while the system is running? Or do I need to look 
during boot, or with a LiveCD diagnostic disk?

Apart from trying to forestall further failures, I've got a spare 
chassis that I'm building as a replacement, this time using CentOS 5. 
Where in the install process should I be installing OMSA or any other 
recommended utility?
