OpenManage 'execute application'

OM is currently configured to execute a mail script (/opt/dell/dellalert.sh) on any warning or failure.
/opt/dell/dellalert.sh
(omreport chassis;omreport system alertlog) | mail -s "Dell Alert on `hostname`!" admin at secure.intranet.domain.com
The script is invoked on temperature warning/failure alerts (I manually decreased the max temp to a setting below the current box temp to trigger a temperature event). The alert email was received a second later.
We had a hard drive warning and failure over the weekend, but OM did not call /opt/dell/dellalert.sh as expected, based on the current configuration.
In the Alert Log overview - every single event has a checkmark beside 'execute application'. Every event references the same /opt/dell/dellalert.sh script.
What would cause OM to intermittently invoke alert based applications?
