Hi,

Did the problem appeared since you update your kernel from yum update ?
If so, please find a supported PowerPath for your new kernel (5.2 or 5.3) and reinstall or update it.

Problem should be due to fact update 3 replace QLogic driver with native ones and disable PowerPath module as it is a kernel dependant one.
You can eventually use dm-mpio too on RHEL 5.3 (in place of PowerPath).
