Kernel error on CD-RW/DVD ROM

I have a 2950 running RHEL4.7 that has been getting the following errors
in its logs.  They had been rather infrequent - every 3-6 months, but
have been occuring more often lately and are now daily.  /dev/hda is my
CD-RW/DVD ROM drive and is not in use when these messages are generated.

I've searched the net several times, but have not found anything similar
enough to give me an idea as to how to address this issue or what level
of concern to give it.  We don't use OMSA and generally don't upgrade
firmware unless necessary.  The server also has a Perc 5/i controller
for the internal SAS drives, RAID 5 configuration.  It is a busy
production server.  I'm looking for any idea that will help me
trouble-shoot this.
