I had the exact same issue:

and as I told in that thread I only managed to resolve by booting with the
CentOS live-cd you can find here
http://linux.dell.com/files/openmanage-contributions/omsa-51-live/
and then using the standard RedHat bin file you are using as well. Which
distribution are you using, just for curiosity's sake?
