Input Outpout Error

I built the first LUN with 50G with (/dev/sdb, /dev/sdd) then I
expanded with another 50G (/dev/sdc, /dev/sde)
the expansion done without error but i still have the error i/o ...
I get this when doing pvscan, vgscan, and
lvscan, lvdsipaly, fdisk -l for example:
