So, this turns out to be *NOT* a squid problem, but rather a likely
urllib or up2date problem. It looks like up2date is mis-parsing urls
when they have double slashes in them. I have 'fixed' all of the mirror
scripts (the software/mirrors.pl, hardware/mirrors.pl, and fwupdate.com
mirrors.pl) to cleanse the path so that they never return double
slashes.

I suspect that this problem only manifests on proxy setups, otherwise my
testing would have uncovered this much earlier.

My thanks to Grant for sending me the necessary data to diagnose this
and get the fix.
