And do you get any alert in OMSA about the disk going to removed or
missing or something?
If the OMSA Alert log doesn't contain anything then a copy of the
controller log would be helpful.
(omconfig storage controller action=exportlog controller=0)

thanks for all reply's on this, both on and off list.

Nothing in OMSA about this apart from when the disk goes awol and 
nothing smart related. Details of the disk in question after a rebuild 
below but again nothing untoward.
