2950 - PERC5 - 'Disk Removed'

That actually wouldn't cause the drive to go removed. Sounds like a
possible Bad Phy problem. Are you getting any alerts in OMSA around the
time that the drive exhibits this behavior?

Any SMART indications from the drive in question?
