To prevent something similar from happening again:

Vendor ID                 : MAXTOR
Product ID                : ATLAS10K5_73SCA
Revision                  : JNZM

... you might want to upgrade your disk-firmware; JT00 is the "latest
and greatest" for this disk and that release was an Urgent one, I believe.
Not sure if the issue in your current firmware can be the cause of a
Failure like you've seen, but I've seen arrays degrade because of this.

Fortunately you had this disk in a RAID1 apparently so the array was
just "degraded" and there was no big disaster :-)

P.S. I can't tell from the output you sent; but of course make sure your
RAID-controller firmware is up-to-date too ;-)
