PowerEdge 2650 ext3_get_inode_loc errors

Follow-up:  This only happens with CentOS 5.1, not CentOS 5.0 - but
again, only on this particular machine.

I have seen posts regarding this particular error, but have yet to see a
solution for it yet.  I have deployed several (10+) PowerEdge 2650's
with PERC 3/DI adapters successfully running CentOS5-5.1.  This 2650 is
giving me errors.  The PERC is running the most current firmware (2.8-1
Build 6098).  The installation goes fine, but the first boot fails with
the following error:

 

EXT3-fs error (device sd (8,2)): ext3_get_inode_Loc: unable to read
inode block

 

Has anyone successfully diagnosed this problem?  Like I said, I have
several other PE2650's using the same PERC/firmware with no problems.
The error occurs at every boot and effectively brings the system to a
halt.
