Afacli really won't give you anymore information. OMSA may be able to,
however depending on what state the drive is in even OMSA might not be
able to determine the problem anymore.

As for repairing the logical drive you just need to replace the failed
disk. If you want to actually repair the physical disk... Well that is
rarely possible and generally more trouble than it is worth.

Hi! I have a PE2650 with controller PERC 3/Di and Linux 2.4.29 as OS.
One day the server failed and all the operations related with the SO
like ls, mkdir, cd ... return an error code Input/Output Error. I
rebooted the server and it worked, but I have  a led of one disk with
orange colour.

I've installed the application afacli and I've seen this results:
