Adaptec PERC2 going offline on Dell PE 8450

Hi All,

We have a PE 8450 runing mostly Debian woody, some backports, on a 2.4.27 
kernel (built from debian kernel source package kernel-source-2.4.27 2.4.27-5).

All the system partitions are on a PERC2 card RAID 1 volume using the two 
internal hot swap disks.

Periodically, the controller gets taken offline by the kernel with the 
following log messages:

Followed by lots more xfs and I/O errors as you might expect.

I recently flashed the card to firmware v.2.8.0.6089, A13 to try and fix this 
problem to no avail as you can see.

Is there a known solution to this problem?
