Jim,

Looks like your kernel is claiming due an I/O problem in your server.
Probably if you have Perc3/DI .....not again....:(

But well, to have a better look into this and avoid confusion..

-Use RHAS Support, if you are already paying license and maintenance
 :/

-Place some extra info, provide by ksymoops.
 Run it with no parameters and paste the error you already copy from 
your /var/log/{message,kernel}. You will have some back trace output , it 
will be useful for kernel hackers to take a look and give an opinion,
(you can post it into the right mail-list)

If you don't want to bother the rest people in this list with your 
attachment, send only to me. I had some issues with kernel 2.6. in a lab with a 
PE2650, so probbaly I can help you (not sure, but I will try)

Some Qs.
What BIOs version are you using?
What about Perc Bios firmware?
