You can try my local RPM
http://yum.telkom.net.id/dkms/dkms-bnx2-1.5.10c-1.fan.noarch.rpm
It's created from source package on Broadcom's site

Eh? How come it failed? I'm using it on RHEL5/xen, also with kernel
2.6.18. It builds and loads perfectly with dkms. Admittedly, I haven't
try using it to replace kernel's source files directly. I also noticed
that there's another file on this version, bnx2_fw2.h, that you didn't
mention. You might want to give it a try.
