For what distribution?  DKMS can build Red Hat, SuSE, and Ubuntu
driver disks.  The code there is about the only documentation I've
found.  http://people.redhat.com/dledford has some really really old
documentation for the early Red Hat format; it's changed somewhat
since then, so I tend to read the Red Hat/Fedora anaconda loader
source to see what it really is looking for.  Ubuntu has some decent
docs in their wiki under the kernel pages (wiki.ubuntu.com).
