Kevin, that is correct. We had a packaging bug with keystone (rcb14) where all patches were rolled back. We pushed out rcb15 which has all of the patches applied.

Keystone server.rb chef recipe is depending on the output from keystone-manage utility to achieve the idempotency (e.g. it simply greps some_data:

But this ISO image consists of 2011.3-final-rcb14 packages that seem to be outdated, because "keystone-manage user list" doesn't output user names at all, only ids.
Also, I've checked 2011.3-final-rcb15 packages. And yes, they output user names and other things as expected in the keystone recipe.
So, I would highly recommend you to rebuild the ISO with the latest RCB packages.
